Pricing is transparent, but please note services are not offered À la carte, as chiropractic care is tailored to your specific needs and recommended based on your history and exam. Most adjustments are supplemented with some form of soft tissue therapy, exercises, ergonomic or postural recommendations, and/or active home care advice. Treatment typically costs $45-60, with additional exam fees if applicable.
Established patient exam fees apply for new complaints or if you haven’t been seen in over three months, while patients absent for more than three years are considered new with applicable fees.
Cancellation Policy
Your appointment time is reserved just for you. A late cancellation or missed visit leaves a gap in the provider’s schedule that could have been filled by another patient. Therefore, we require 24 hours’ notice for any cancellations or reschedules. New patients wishing to reschedule after a late cancellation or no-show will be charged a $30 fee, and established patients will be charged a $15 fee per occurrence after 2 “passes” per year. Billing and Payment Policies
Insurance – In Network
I am currently contracted with Premera, LifeWise, Asuris Northwest Health, HMA, Regence BlueShield, First Choice Health Network, Kaiser, PacificSource, and Providence Health Plan. I bill these plans directly and collect any applicable copays, deductibles, or coinsurance.
If you have one of the insurance plans listed above but prefer not to bill insurance, please let me know, and we will complete the necessary documentation at the time of your visit.
Insurance – Out of Network
I may be able to check your eligibility and benefits for payors not listed, and I will do my best to inform you of your coverage. For out-of-network plans, I do not bill your insurance, and you are responsible for any payment due. However, I can provide you with a superbill, which you can submit directly to your insurance company for potential reimbursement.
Other Billing
If you are paying out-of-pocket for services, payment is preferred at the time of service. Accepted methods of payment include cash, card, check, Venmo, or HSA/FSA cards.
Please let me know if there are extenuating circumstances preventing you from making payment on the same day treatment is rendered. We may be able to work out a payment plan, but I reserve the right to discontinue care in the case of nonpayment for services.
I am currently in the process of enrolling as a new Medicare provider and am not yet able to see patients with Medicare.
Lastly, please let me know if your treatment is related to an open workers’ compensation, motor vehicle, or other injury claim where a third party is responsible for payment. Payment may be due at the time of service.
